2em">根据《陕西省2024年下半年中小学教师资格认定公告》和《渭南市教育局关于做好2024年中小学教师资格制度实施工作有关事项的通知》（渭教师〔2024〕19号）要求，结合我县实际，组织开展2024年下半年中小学教师资格现场确认工作。现将我县教师资格现场确认事项公告如下：</span></p><section style="text-indent: 2em;line-height: 16px">一、受理范围</span></strong></span></p><section style="text-indent: 2em;line-height: 1.75em"><span style="font-size: 16px">获得教育部考试中心颁发的《中小学教师资格考试合格证明》且在有效期的人员。 </span></section><section style="text-indent: 2em;line-height: 1.75em"><span style="font-size: 16px">（一）户籍在富平县的社会人员（含在职、待业人员）。 </span></section><section style="text-indent: 2em;margin-bottom: 16px;line-height: 1.75em"><span style="font-size: 16px">（二）持有富平县辖区居住证并在有效期内的外省市户籍人员。</span></section><p style="text-indent: 2em;line-height: 1.75em;margin-bottom: 8px"><span style="text-indent: 2em;background-color: rgba(255, 255, style="text-indent: 2em;line-height: 1.75em"></section><section style="text-indent: 2em;line-height: 1.75em;margin-bottom: 8px"><span style="font-size: 16px">（一）提交个人材料（自带档案袋）</span></section><section style="text-indent: 2em;line-height: 1.75em"><span style="font-size: 16px">1.身份证明 </span></section><p style="text-indent: 2em;line-height: 1.75em;margin-bottom: 8px"><span style="font-size: 16px">二代身份证（需在有效期内）原件和本人户口簿或集体户口证明原件（现场审核完返还本人）、富平县辖区居住证（外省市户籍人员）。</span></p><section style="text-indent: 2em;line-height: 1.75em"><span style="font-size: 16px">2.学历证明 </span></section><section style="text-indent: 2em;line-height: 1.75em"><span style="font-size: 16px">学历信息通过“教师资格管理信息系统” 核验的不用提交学历证书原件。不能核验的需提供学历证书原件和学历鉴定证明（学信网在线申请毕业证书电子注册备案表）。</span></section><p style="text-indent: 2em;line-height: 1.75em;margin-bottom: 8px"><span style="font-size: 16px">持港澳台地区高等学校学历学位证书和国外高等学校学历学位证书的申请人，需提供教育部留学服务中心出具的《国（境）外学历认证书》原件，仅此学历学位认证证明视为有效。 </span></p><section style="text-indent: 2em;line-height: 1.75em"><span style="font-size: 16px">3.普通话等级证明 </span></section><p style="text-indent: 2em;line-height: 1.75em;margin-bottom: 8px"><span style="font-size: 16px">本人普通话水平等级测试信息通过“教师资格管理信息系统”核验的不用提交普通话证书原件。不能通过核验的需提供普通话证书原件。 </span></p><p style="text-indent: 2em;line-height: 1.75em;margin-bottom: 8px"><span style="font-size: 16px">4.近期免冠正面1寸彩色白底证件照2张(</span><span style="font-size: 16px;color: rgb(0, 122, 170)"><strong>应与网上申报及体检表时相片同底版，照片背面注明姓名、申请学科、身份证号和联系电话</strong></span><span style="font-size: 16px">)。 </span></p><p style="text-indent: 2em;line-height: 1.75em;margin-bottom: 8px"><span style="font-size: 16px">5.申请认定中等职业学校实习指导教师资格的人员，除提供以上资料外，还需提供专业技术职务证书或工人技术等级证书原件。 </span></p><p style="text-indent: 2em;line-height: 1.75em;margin-bottom: 8px"><span style="font-size: 16px">以下几种情况不受理申请和不接收材料：（1）不在认定范围内的；（2）不符合认定条件的；（3）证件不齐全或弄虚作假的（教师资格认定机构将对申请人提交的学历证、普通话等级证等进行核查，发现故意造假，骗取教师资格的，将按《教师资格条例》第十九条规定处理：弄虚作假、骗取教师资格的，自撤销之日起5年内不得重新申请认定教师资格）。 </span></p><section 16px">1.体检标准严格按照陕西省教育厅、陕西省卫生健康委员会《关于做好陕西省教师资格认定体检工作的通知》（陕教〔2019〕61号）有关规定执行，按照省市要求，体检医院为二级甲等以上医院，统一使用《陕西省申请认定教师资格人员体检表》。 </span></section><section style="text-indent: 2em;line-height: 1.75em"><span 1.75em"><span style="font-size: 16px">4.体检注意事项： </span></section><section style="text-indent: 2em;line-height: 1.75em"><span style="font-size: 16px">（1）体检人员首先要通过教育局现场认定通过后，领取教育局粘贴本人照片（带一张近期免冠二寸照片）并盖章的体检表，随后去指定医院进行体检。 </span></section><section style="text-indent: 2em;line-height: 1.75em"><span style="font-size: 16px">（2）体检当天早上需空腹（即不喝水、不吃早餐），凭身份证、《体检表》（加盖公章）参加体检。体检结束后，务必在当天把《体检表》交回医院。 </span></section><section style="text-indent: 2em;line-height: 1.75em"><span style="font-size: 16px">（3）体检报告由指定医院直接移交给各确认点，体检结果不逐一电话通知，如在体检后15个工作日内没有接到电话通知，证明通过体检。 </span></section><section style="text-indent: 2em;line-height: 1.75em"><span style="font-size:
